The ideal candidate will provide high-quality, patient-centered care to individuals with life-limiting illnesses, supporting both patients and their families through end-of-life care.**Key Responsibilities:**- Deliver skilled nursing care to hospice patients in accordance with physician orders and established care plans.- Assess patient needs, develop individualized care plans, and update as necessary.- Provide emotional support and education to patients and their families regarding disease progression, symptom management, and end-of-life care.- Administer medications, monitor patient symptoms, and collaborate with interdisciplinary teams to ensure comprehensive care.- Maintain accurate and timely documentation of patient care and progress.- Participate in on-call rotations and respond promptly to patient needs.**Qualifications:**- Valid Registered Nurse (RN) license in the state of Kansas.- Minimum of 1 year of nursing experience, preferably in hospice, palliative care, or home health.- Strong clinical assessment and critical thinking skills.- Excellent communication and interpersonal abilities.- Compassionate, patient-focused approach to care.- Ability to work independently and as part of a multidisciplinary team.- Current CPR certification.**Benefits:**- Competitive compensation and benefits package.- Opportunities for ongoing training and professional development.- Supportive and collaborative work environment.- Access to innovative tools and resources.
