About the Role:

We are seeking a dedicated Nurse Practitioner to join a collaborative clinical care team. The provider will assess patient healthcare needs, deliver high-quality patient care, and support continuous improvement initiatives within a fast-paced healthcare environment. This role focuses on patient-centered care, teamwork, and operational excellence.

Key Responsibilities:

• Evaluate and treat patients according to established standards of care
• Coordinate patient services including specialty referrals, hospital coordination, home healthcare, and medical equipment support
• Participate in daily care team discussions and collaborate with clinical staff
• Maintain accurate and timely medical documentation
• Follow clinical policies, procedures, and care protocols
• Support quality improvement and patient outcome initiatives
• Assist with new and existing healthcare service growth opportunities
• Build positive relationships with local healthcare providers and community organizations
• Perform additional duties as assigned

Qualifications:

• Completion of a Nurse Practitioner
• Strong communication and interpersonal skills
• Ability to work in a collaborative, team-based healthcare environment
• Comfortable adapting to value-based care practices
• Basic computer knowledge including EMR systems and email
• Strong patient care and organizational skills

Work Environment:

• 100% onsite role at the care center location
• May require travel between nearby clinic locations
• Flexible schedule including morning, evening, or weekend shifts as needed
