Job Description

Job Title: Hospice RN / RN Case Manager

Position Overview:
As a Hospice RN / RN Case Manager, you will serve as the primary point of contact for patients and their families, guiding them through every step of their hospice journey. You will deliver skilled, compassionate nursing care based on individualized Plans of Care, whether in the patient’s home, assisted living, or other settings. This role requires strong clinical skills, empathy, and the ability to collaborate with an interdisciplinary team to ensure the highest quality of end-of-life care.

Key Responsibilities:

  • Serve as the main liaison for patients and families, providing guidance and support throughout the hospice experience.
  • Deliver and document hands-on nursing care according to each patient’s individualized Plan of Care.
  • Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
  • Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to develop and refine care plans.
  • Provide emotional, physical, and spiritual support to patients and their loved ones, including education, comfort, and bereavement guidance.
  • Educate caregivers on safe and effective care practices, offering both practical training and emotional reassurance.
  • Communicate important updates in patient condition to the care team for timely interventions.
  • Maintain accurate, up-to-date clinical documentation and adhere to infection control and compliance standards.
  • Support quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care.
  • Help patients and families understand and navigate medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes.

Qualifications:

  • Active RN license in the state of employment (or eligibility to obtain).
  • Minimum 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
  • Compassionate, empathetic, and patient-centered approach to care.
  • Comfortable providing care in diverse settings, including private homes and facilities.
  • Strong communication and critical thinking skills, especially in emotionally sensitive situations.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • CPR certification required.

Preferred Experience (Not Required):

  • Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
  • Admissions RN, wound care, float pool, or travel nursing experience.
  • Experience supporting terminally ill patients or those with serious illnesses.
