Job Title: Hospice RN / RN Case Manager
Location: United States (Remote/Field-Based)
Position Overview:
As a Hospice RN / RN Case Manager, you will serve as the primary point of contact for patients and families, guiding them through every step of their hospice journey. You will deliver skilled, compassionate nursing care in patients’ homes, assisted living facilities, or wherever they call home. This role requires a blend of clinical expertise, emotional intelligence, and a genuine commitment to patient-centered care.
Key Responsibilities:
- Serve as the main liaison for patients and families, providing guidance and support throughout the hospice experience.
- Deliver and document hands-on nursing care based on individualized Plans of Care.
- Perform initial and ongoing assessments to monitor patient conditions and adjust care as needed.
- Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to develop and refine personalized care plans.
- Provide emotional, physical, and spiritual support to patients and their loved ones, including education, comfort, and bereavement guidance.
- Educate caregivers on safe and effective care practices, offering both practical training and emotional reassurance.
- Communicate important updates in patient condition to the care team to ensure timely interventions.
- Maintain accurate, up-to-date clinical documentation and adhere to infection control and compliance standards.
- Support organizational quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care.
- Help patients and families understand and navigate medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes.
Qualifications:
- Active RN license in the state of employment (or eligibility to obtain).
- Minimum 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
- Compassionate, empathetic, and patient-centered approach to care.
- Comfortable providing care in diverse settings, including private homes and facilities.
- Strong communication and critical thinking skills, especially in emotionally sensitive situations.
- Valid driver’s license, auto insurance, and reliable transportation.
- CPR certification required.
Preferred Experience (Not Required):
- Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
- Admissions RN, wound care, float pool, or travel nursing experience.
- Experience supporting terminally ill patients or those with serious illnesses.
