We are seeking a compassionate and dedicated Hospice RN / RN Case Manager to join our team. In this role, you will be the primary point of contact for patients and families, guiding them through every step of their hospice journey. You will deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home. Responsibilities include performing initial and ongoing assessments, collaborating with an interdisciplinary team, providing emotional, physical, and spiritual support to patients and their loved ones, teaching caregivers, communicating important updates to the care team, maintaining accurate clinical documentation, and supporting quality improvement initiatives. You will also help patients and families understand medication administration, hospice philosophy, symptom and pain management, and end-of-life processes.

Qualifications:

• Active RN license in the state of employment (or eligible to obtain).
• 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• Compassionate, empathetic, and patient-centered approach.
• Comfortable providing care in diverse settings, including private homes and facilities.
• Strong communication and critical thinking skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• CPR certification required.

Preferred Experience:

• Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
• Admissions RN, wound care, float pool, travel nursing
• Experience with terminally ill patients or serious illness support
