Hospice RN / RN Case Manager

About the Role:
As a Hospice RN / RN Case Manager, you will serve as the primary point of contact for patients and their families, guiding them compassionately through every step of their hospice journey. You will deliver skilled, hands-on nursing care based on individualized Plans of Care, whether in the patient’s home, assisted living, or other settings. Your role is pivotal in ensuring comfort, dignity, and support for patients and their loved ones during end-of-life care.
Key Responsibilities:
- Serve as the main liaison for patients and families, providing guidance and support throughout the hospice experience.
- Deliver and document skilled nursing care according to each patient’s individualized Plan of Care.
- Conduct initial and ongoing assessments to monitor patient conditions and adjust care as needed.
- Collaborate with an interdisciplinary team (physicians, LPNs, CNAs, social workers, chaplains, etc.) to develop and refine personalized care plans.
- Provide emotional, physical, and spiritual support to patients and their families, including education, comfort, and bereavement guidance.
- Educate caregivers on safe and effective care practices, offering both practical training and emotional reassurance.
- Communicate significant changes in patient condition to the care team for timely interventions.
- Maintain accurate clinical documentation and adhere to infection control and compliance standards.
- Support quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care.
- Guide patients and families on medication administration, hospice philosophy, symptom and pain management, and end-of-life expectations.
Qualifications:
- Active RN license in the state of employment (or eligibility to obtain).
- Minimum 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related fields.
- Compassionate, empathetic, and patient-centered approach to care.
- Comfortable providing care in diverse settings, including private homes and facilities.
- Strong communication and critical thinking skills, especially in emotionally sensitive situations.
- Valid driver’s license, auto insurance, and reliable transportation.
- CPR certification required.
Preferred Experience (Not Required):
- Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
- Admissions RN, wound care, float pool, or travel nursing experience.
- Experience supporting terminally ill patients or those with serious illnesses.
